Using our Address Book template is a quick way to be able to add multiple people's contacts to your FOREVER® Address Book at once. You can find the template spreadsheet at the end of this article to download and edit!
Quick Tips
- Keep the first line unchanged - The very first row of the spreadsheet with headers (such as First Name, Last Name, etc.) should remain unchanged. This is how our system recognizes which column to find certain information for the addresses you want to add.
- Stay within 50 characters per cell - Try to shorten or abbreviate names and addresses as much as possible to ensure it fully fits on the envelope printing.

Fill in at least the First or Last Name - You have the option to fill in a First Name and Last Name for the contact you're entering the address for. In order for our system to create a contact's profile, at least one needs to be filled in.
This means you can have First Name only, Last Name only, or First and Last Name. If neither First nor Last Name are filled in then a contact will NOT be created. - There is NO duplicate detection - This means if you import your filled template more than once it will create multiple copies of the same address in your Address Book. We ask that you triple-check the addresses in your template before upload, then make necessary adjustments by adding or editing the existing addresses here. Alternatively, you can Reset your Address Book to completely delete all addresses at once, and re-upload the filled template to completely avoid duplicates from the last upload(s).
- Special characters accepted - You can use special characters such as & # ! ( ) in any of the template's cells.
Template Columns Explained
There are 9 different columns you can fill with information about the contacts you want to add to your account's online Address Book. Here is a list of the columns and their overall purpose.
Name
⚠️REMINDER: You must have at least First -OR- Last filled in for a contact to be created.
| First Name | a single person's or multiple people's first names | "Jane" or "Jane & Joe" |
| Last Name | the last name of the contact, it can be singular or hyphened | "Smith" or "Johnson-Miller" |
| Household Name | a way to name the entire home the address is for | "The Smith Family" or "Johnson-Miller Home" |
Address
| Address 1 | the first line of an address, usually the building number and street name | "One PPG Place" or "123 Apple St" |
| Address 2 | the second line of an address, further identifying information to distinguish someone's residence or mailbox in a multi-place complex | "20th floor", "apt 2102", or "PO Box 90" |
| City | specific city for the contact's address | "Pittsburgh" or "Calgary" |
| State/Province | this area should be an abbreviation of the state or province, instead of the fully spelled out name | "PA" for Pennsylvania or "AB" for Alberta |
| Zip/Postal Code | series of letters and/or digits added to a postal address to facilitate mail sorting, routing, and delivery efficiency | "15222" or "N9A 7A2" *see below if it start with zero |
| Country | optional unless you are mailing your cards out of the country you currently live in, we recommend this is also an abbreviation instead of fully spelled out | "USA" for United States of America or "AUS" for Australia |

Microsoft Excel
- Right-click at the top on the letter in the Postal Code Column
- Select "Format Cells" from the menu
- Go to the "Number" tab
- Select "Category: Special"
- Choose "Type: Zip Code"
- Select "Ok"

Numbers on Mac
- Click the letter above the column to select everything
- Select the Format (paint brush) button in the upper right corner
- Choose "Cell”
- Under "Data Format" choose "Text"
Importing an Address Book Template
Once you have your addresses filled out in the template (get from the download below) you will want to make sure you save/export it as a CSV file. While logged in to your FOREVER account...
- Hover over the person icon in the upper right (or click your circle profile photo/initials)
- From the menu list select "Address Book"
- Click the button to "Import CSV"
- In the pop-up select "Upload Your CSV"
It may take a few moments for all your contacts to import and have their addresses listed — please do NOT refresh your screen or navigate away from this page while it is processing
After they are uploaded, you can add them to Groups or even edit them as needed!
