Online Project Estimator for Digitization

Is it possible to digitize the project estimator form that comes with digitization boxes? It could be a normal link that people can go to and fill in all of the info themselves or it could be a QR code on the side of the box (or just a regular code that people put into the website) that has the "tombstone data" and box size already filled in. People could then have the option to print out the form once complete or have a place on the form where the number on the side of the digitization box can be added to the form before it is emailed or automatically logged through the website. I think this would give people a much better estimate of how much the box is going to cost. It would also save significant time for folks like me who think it is a good idea to pack 4 boxes at once, each with LOADS of photos and specialty products :)
